total [minimum] embedment allowing for masonry poured concrete

How to assess the embedment depth of an expansion/friction type anchor into a concrete grout filled masonry block pier? The anchor reaction is tension and shear. Masonry 50mm thick.
A recent failure had the anchor embedment depth within the masonry and pulled the masonry away from the grout.
The anchor didn't fail.
With a deeper anchor, the failure cone will reasonably form from the end of the fixing with a finite compressive face area at the inner face of the masonry.
How deep into the grout?
